How Do I Get Started?
Are you eligible? You must be:
• Financially eligible for Medicaid
• Be moderately at risk for placement in a nursing home or facility
• Need hands-on personal care
• Have physicians care for chronic illness or physical or mental disability
• Have an approved certificate of need

How often will a Personal Care Provider visit? This is determined by the assessment performed by a nurse case monitor from the local health department. See Baltimore Department of Health & Aging for more information.

Depending on the assessment, services may be provided from one to seven days a week. Once the assessment is completed the nurse case monitor will develop a plan of care with the recipient, and the appropriate training for the provider will be arranged.

What can you tell me about your caregivers, aides or companions? Home care aides are supervised by a registered nurse or nurse case monitor. Our aides and companions are chosen based on their professionalism, enthusiasm and compassion for seniors and adults.They are screened based on the standards set by the Agency for Healthcare Administration.

How Does the Matching Process Work?
We pride ourselves on matching clients with thoroughly screened, bonded and insured caregivers. The clients design their own schedules in daily, weekly and/or live-in arrangements.
Can I meet the home care aide before they arrive?
Absolutely! Our goal is to build a wonderful relationship between clients and our caregivers or aides. We will discuss the needs and personality of your loved ones and match them to the right caregiver.

Skilled Nursing Services such as those provided by a registered nurse, are not part of this program.
